Dear Sandy,
When you called yesterday to ask me how I liked our VistaKEANE system, I only had time to say, "it's great." I know how hard it can be to choose a new system, so I'm sending this e-mail to tell you how we use it every day...

Pre-Admit
Brian Thomas called Friday. He said his mother had broken her hip and would be needing skilled care. We entered basic information on Mrs. Thomas in Admit Discharge Transfer software.

Admit
When Central Hospital phoned our Admission Coordinator, she pulled up Mrs. Thomas’s pre-admit record and added information to it for the face sheet. We printed our questionnaire for determining level of care from User-Defined Assessments.

Assessment
After she was settled in her room, I talked to her doctor and typed the orders for medications and rehabilitation into our Physician Orders software. That afternoon we printed the orders and sent them to Dr. Johnson for signature.

The Physical Therapist completed his initial assessment and entered his findings plus his treatment plan in VistaKEANE Rehab.

It’s easier to score and track the Falls Assessment since we entered the paper form we had been using for years in the User-Defined Assessment application.

We finished the 5-day MDS assessment yesterday and ran it through Case Mix. I give our Medicare biller the results, with RUG score, reference date, and reason.

Planning
I evaluated the MDS RAPS and got a good start on her care plan. After lunch, I’ll add more information, using the MDS library in our Care Plan software.

The nursing aides refer to the Kardex for how much assistance Mrs. Thomas needs with bathing. It just takes a second to produce, now that the software fills it in from the MDS responses.

At the care plan conference we reviewed her Cognitive Performance Score and we discussed the problem of impaired decision-making. We decided that it is resolved, now that she’s more comfortable here.

Provide Care and Evaluate
It took me a while to get used to typing my progress notes into the software, but now it takes me only a fraction of the time, especially since I started using the copy-and-paste feature. It’s great too, to look up other discipline’s notes so easily.

I looked at the MDS Due Date report and I’ll start Mrs. Thomas’ next one Monday.

We reviewed our most recent Performance Measures Report at a meeting. You can really see now that our team’s work is reducing new pressure ulcers.

When the surveyors were here today they complimented us on the accuracy of our HCFA 672 and 802 forms. They hadn’t seen software before that allowed updating between MDS assessments.
